Referrals: We accept referrals via telephone, mail and fax: from individuals/self referrals, family members, doctors, nurses, social  workers, police, first response staff, hospitals.

Fee: Our services are free-of-charge

Accessibility: We are wheelchair accessible

Availability:

  • Accessing the services of a nurse can take between   1 and 5 days.
  • Obtaining an appointment with a psychiatrist can take between 7 and 30 days.
Who we are: Six Nations Mental Health Services is dedicated to Promoting mental wellness of the individual, family and community.

Our goals:

  • To provide mental health crisis response to Six Nations community members.
  • To provide case management services to clients experiencing mental illness.
  • To provide an opportunity for adult and child psychiatric assessments and consultations in a community integrated clinical setting.
  • To provide an advocacy role for clients and family members who are seeking various services in and out of the community.
  • To promote and enhance individual and family awareness, education and understanding of mental health.
  • To provide culturally relevant care to registered band members residing on Six Nations Grand River Territory.

What we offer:
Services Offered/Clientele that may benefit from our services:

  1. Crisis Response Services for:
    • Serious mental health illnesses
    • An individual experiencing a crisis
    Staff are available on an immediate/temporary basis. Services can be accessed over the telephone to discuss your needs. Office or home visits are possible. Following an initial assessment appropriate services will be implemented as required.
  2. Support Services for clients, family, community members and agencies:
    • Education: Age appropriate and culturally relevant in-school mental health curriculum delivery.
    • Workshops: Mental illness, self-esteem, well-being, stress management, self-awareness, coping skills, respect, diversity and tolerance...
    • Resource Material: Books, videos, and information packages on specific illnesses.
  3. Case Management available to individuals experiencing a serious mental health illness and psychosocial problems:
    • Initial contact includes orientation to agency services; consent/release forms; psychiatric assessment (past and present mental, emotional, physical and spiritual).
    • Psychotropic medication monitoring
    • Monitoring of clients holistic well-being
    • Advocacy and referral to appropriate organizations/services
    • Health education for clients and families
  4. Psychiatric Care:
    Adult Psychiatrists available for:
    • Assessments
    • Consultations
    • Monitoring of medication
    • Long term care and follow up for clients with a serious mental illness
    Child Psychiatrist available for:
    • Assessments
    • Consultations
  5. Supportive Housing:
    To enhance self-sufficiency and enable empowerment, in a community-like setting. We have a limited number of housing units available to our clients experiencing psychosocial difficulties/or mental health illness. Geared towards income, these units are safe, modern and close to amenities in the village of Ohsweken.
