Referrals: We accept referrals with the agreement and involvement of the family doctor from:

  • Community health professionals
  • Health care facilities
  • Self or family

Admission criteria: 

  • Over the age of 65 or having an age-related mental health problem.
  • Over the age of 50 for complex mood disorders.

Fee: At the moment the only programs with small fees are the Staywell Program and the Day Hospital. Fees are subject to change.

Accessibility: Wheelchair accessible.

Availability: There is usually a short delay period for our services. Please call for more information and estimated waiting time.

Who we are: HOAP is a combined medicine and psychiatry program located in a community ambulatory care centre in Hamilton's East end. It provides services to older adults with complex health problems. HOAP requires the agreement of a family doctor for referral.

HOAP is comprised of a multidisciplinary team: geriatricians, psychiatrists, nurse case managers, physiotherapist, occupational therapist, therapeutic, recreationist, exercise coordinator, kinesiologist, social worker and pastoral care worker.

HOAP, in keeping with the mental health vision statement of St. Joseph's Healthcare, Hamilton, our purpose is to improve emotional, psychological and spiritual well being for older adults by providing effective, integrated mental health services and serving those we care for with compassion, dignity and respect.

HOAP Community Development: Case managers are assigned to the long term care facilities in the catchment area and act as external resources to PIECES trained personnel.

HOAP Education: HOAP provides a setting for the education of residents, clinical clerks, nursing, physiotherapy, occupational therapy and other students.

HOAP Research: Staff are strongly encouraged to be engaged in evidence-based practice and to be involved in research projects.

  • Consultation, diagnosis and treatment recommendations for psychiatric/mental health disorders, including complex mood disorders.
  • Cognitive Behavioral Therapy -individual and group

  • Education & support for individuals and families
  • Individual and group psychotherapy
  • Positive Actions & Attitudes: an exercise, leisure and health education program for older adults with mental problems.
  • Caregiver Support Group: for family members/significant others caring for people with depression
  • Community partnerships for coordinated, effective, patient-centered care; educational initiatives, and community development
  • Education opportunities for learners
  • Research into helping older adults with mental problems.
