Who we are:
HOAP is a combined
medicine and psychiatry program located in a community ambulatory care
centre in Hamilton's East end. It provides services to older adults with
complex health problems. HOAP requires the agreement of a family doctor
for referral. HOAP is comprised of a multidisciplinary team: geriatricians, psychiatrists, nurse case managers, physiotherapist, occupational therapist, therapeutic, recreationist, exercise coordinator, kinesiologist, social worker and pastoral care worker. HOAP, in keeping with the mental health vision statement of St. Joseph's Healthcare, Hamilton, our purpose is to improve emotional, psychological and spiritual well being for older adults by providing effective, integrated mental health services and serving those we care for with compassion, dignity and respect. HOAP Community Development: Case managers are assigned to the long term care facilities in the catchment area and act as external resources to PIECES trained personnel. HOAP Education: HOAP provides a setting for the education of residents, clinical clerks, nursing, physiotherapy, occupational therapy and other students. HOAP Research: Staff are strongly encouraged to be engaged in evidence-based practice and to be involved in research projects. What we offer: |
