• In the top left corner this scan records the patient name (surname erased here, first name = Cory), his hospital number (1446281), birth date ( 08- Feb- 1986), and the date of the exam ( 26-Jul-2003).

• Different scanners and different techs may record this information in different areas of the scan. However, you should always find at least the patient's name and the date of examination somewhere on the images.
