Answers (a very challenging image with lots of abnormalities!)

1. Septum pellucidum shifted 5 mm to left

2. multifocal

3. both intra-axial (brain) and extra axial (bone, cisterns)

4. Bilateral medial occipital and posterior inferior temporal  infarctions, subacute subarachnoid hemorrhage, left fronto-temporal craniectomy, catheter in right anterior horn, lacunar infarctions left temporal lobe and left insula, mild enlargement of anterior horns, obliteration of sulci, reduced gray-white differentiation. Dx: Subacute subarachnoid hemorrhage with hydrocephalus, ventricular drain, craniectomy, suspected vasospasm causing multiple cerebral infarctions  and with suspected intracranial hypertension.
