1 |
No Response
|
No change in response to any audible, visual, or tactile stimulus;
seems asleep |
|
2 |
Generalized Response |
Eye opening and/or body movement but inconsistent, without purpose,
and not directly related to the type of stimulus given; without gaze
fixation; seems asleep most of the time |
|
3 |
Localized Response |
Response related to stimulus so tracking and fixation of gaze to
visual stimulus, head turning in response to audible stimulus,
sometimes obeys simple command but not consistent. |
|
4 |
Confused, Agitated |
Spontaneously or with minimal stimulus becomes restless, agitated,
possibly aggressive and verbally abusive; may produce incoherent
speech; unable to do self care tasks without help |
|
5 |
Confused, Inappropriate |
Incoherent but not agitated; obeys simple commands consistently; poor
recall, new learning, and concentration, but able to perform
over-learned tasks such as eating |
|
6 |
Confused, Appropriate |
Sensible speech; simple self care possible e.g., brushing teeth though
may need cueing; goal directed but needing direction and supervision |
|
7 |
Automatic, Appropriate |
Coherent, OK for self care; impairments often include: limited recall
of recent events, problem solving, calculations, etc; insight often
limited; learns but more slowly than normal |
|
