Canadian Neurological Scale
The Canadian Neurological Scale was designed to assess neurological function in
conscious stroke patients. It includes an assessment of level of consciousness,
orientation, aphasia, and motor strength. Each domain is assigned a score and a
total score from 0 to 11.5 is calculated. Follow Physician's Orders or Clinical
Path guidelines for frequency of assessing status using the CNS. The CNS Scale
is to be used for alert or drowsy patients.
