Preoperative Testing of the Surgical Patient

Preoperative Testing
All general lab tests and ECG's must be repeated after 90 days. Repeat testing will be done prior to 90 days if previous results are abnormal or specifically ordered by a physician. Group and Screen results must be repeated after 30 days.

Criteria for Patients Requiring an Electrocardiogram:
  1. Recommended for all patients over the age of 60 years
  2. Heart rates greater than 90 and lower than 60 beats per minute (BPM)
  3. Indicated for al patients who possess any of the following:
    1. taking cardiac drugs including blood cholesterol lowering drugs and/or antihypertensives
    2. angina
    3. symptomatic mitral valve prolapse (MVP);
    4. rheumatoid arthritis;
    5. thromboembolic disorders
    6. hypertension
    7. diabetes
    8. cardiovascular disease
    9. renal insufficiency
    10. seizures
    11. shortness of breath
    12. sickle cell disease
    13. carotid bruit
    14. morbid obesity BMI > 35;
    15. cocaine abuse

     

Criteria for Hemoglobin (Hgb) and Hematocrit (Hct) Measurement
  1. patients over the age of 70 years
  2. history of
    1. renal disease
    2. anemia
    3. liver disease
    4. current malignancy/chemotherapy/radiation
    5. bleeding disorders, blood dyscrasia, (any disease of the white cells, red cells or platelets)
    6. hypersplenism
    7. anticoagulant drug use
  3. patients for whom a Group and Screen is ordered
  4. patients booked for major surgical procedures
  5. pediatrics, as ordered by the surgeon or anesthesiologist
 

Criteria for Electrolyte Measurement

  1. History or symptoms of
    1. diabetes
    2. renal disease
    3. organ transplant
    4. hypertension
    5. cardiovascular disease
  2. patients taking the following medications
    1. digoxin
    2. diuretics
    3. steroids
    4. ACE inhibitors
    5. potassim supplements
Criteria for BUN, Creatinine Measurement
  1. 65 years of age and over
  2. history of
    1. cardiovascular disease
    2. diabetes
    3. bleeding disorder
    4. renal disease
  3. patients taking the following medications
    1. digoxin
    2. diuretic use

Criteria for Coagulation Studies (INR/partial thromboplastin time)(INR/PTT):

  1. History of
    1. end stage renal disease
    2. bleeding disorder
    3. anticoagulatn drug therapy - coumadin
    4. liver disease
  2. cardiac/vascular surgery patients
 

Criteria for Livery Function Test:

  1. patients with a history of liver disease or jaundice

Criteria for Human Chorionic Gonadotropin (Urine HCG):

  1. pregnancy status is uncertain in menstruating females
  2. last menstrual period (LMP) is greater than 28 days

Criteria for Chest X-ray

  1. as ordered by physician

Criteria for Urinalysis

  1. symptoms of current urinary tract infection
  2. surgery for prosthetic joint implant
