Craniotomy Cases
The Problem:
Not done yet. 		What We Do:
PRE: Ancef 1g IV on call to OR
Stress dose steroid coverage if on decadron pre-op (Solucortef 100 mg IV on call)
POST: Ancef 1g IV q8h x 3
HOB @ 30 degrees
NPO for first day then increase to sips then DAT
Decadron if on it before
never give hypotonic IV fluids due to the risk of edema
PACU overnight or to NOA

We usually take the drains out POD #1 unless they are for a chronic subdural. If there is any new neurological deficit post op or any decrease in the LOC post op always get a CT head to assess for post op bleeding.
