- All ASA Class III or IV patients
- Patients over 70 years of age.
- All Insulin Dependent Diabetic patients.
- There is uncertainty whetehr the general anaesthetic patient is an ASA Class
II or III
- Any condition limiting flexion and extension of the neck or mobility of the
jaw.
- The patient gives a history of:
- difficult with anesthesia in the past i.e. difficult intubation
- diagnosis of malignant hyperthermia;
- unscheduled Intensive Care Unit (ICU) admission post-operatively;
- muscle weakness (hours) post-operatively;
- any adverse cardiac or respiratoyr event i.e. cardiac arrhythmia
- awareness during a previous general anaesthetic
- heartburn or acid regurgitation or hiatus hernia
- bleeding disorder
- high opiate use (i.e. morphine 60 mg/day)
- The pulse rate is irregular or less than 60 in non-athletes or greater than
90 after repeating at least once before an ECG is done.
- The patient's ECG is abnormal.
- 9. The pateint is on any monoamaine oxidase inhibitors (MAO), St John's Wort,
Yohimbie, or Passion Flower.
- The patient is taking oral steroids and/or anticoagulants.
- The patient is morbidly obese as calculated using the body mass index (BMI
> 35) as a guideline.
- The patient is pregnant.
- The patient is diagnosed with sleep apnea.
- Paedriatrics:
- a) any child age 6 or under
- b) dental restorations
- c) any major surgeries
- d) all neuro surgery
- e) ASA II, III and IV

- The patient is booked for any of the following surgeries:
- General Surgery
- major gastro-intestinal surgery;
- hiatus hernia repair (fundoplication)
- portovenous decompression
- thyroid surgery
- Urological Surgery
- cystectomy
- ileal conduit
- nephrectomy
- postatectomy (radical)
- transurethral resection of prostate
(TURP)
- Neurosurgery
- craniotomy
- cervical spinal surgery
- lumbar or thoracic laminectomy (3 levels or more)
- Orthopaedic Surgery
- major spinal surgery
- total joint replacement surgery
- cervcial spinal fusion
- major orthopedic oncological surgery
- Cardiac Surgery
- all cardiac surgery
- Vascular Surgery
- all arterial vascular surgery
- Gynecology/Oncology
- pelvic exenteration
- radical hysterectomy
- defulking ovarian cancer
- ENT
- thyroidectomy
